We strive to ensure that our service is accessible to all and that we do not inadvertently discriminate against members of any particular group in society. We also respect our obligations under the Equality Act 2010 and the Equality Act (Statutory Duties) (Wales) Regulations 2011.

One of the ways to help us measure how we are meeting these commitments is to undertake equality monitoring.  Equality monitoring is the process of gathering equality information and analysing this information in order to identify areas for improvement.

Your participation is completely voluntary. However, your responses will enable us to better understand who complains to us, what are their diverse needs and how we can improve our service moving on.

Your responses will be held in the strictest confidence. Any information you provide will be held in line with the requirements of the Data Protection Act 2018 and processed only for the purpose of collating and analysing equality statistics.

When you complete this form, we ask you for your case reference number.  We do this, because it allows us to better analyse the accessibility of our service – for example based on the subject or type of complaints.

Please be assured that your response will not be seen at any point by any member of staff responsible for assessing or investigating your complaint.  Only the staff who are undertaking the analysis will see it.

Some questions may feel sensitive or personal. However, we are obliged to ask them under the Equality Act.  If you are not happy to answer any particular question, just select the ‘prefer not to say’ option.

We are using SurveyMonkey for this questionnaire. As part of the service, your responses will be transferred securely to the United States under the EU-U.S. Privacy Shield Framework and held by SurveyMonkey. For further information, please see the SurveyMonkey Privacy and Legal pages here.

Following receipt from SurveyMonkey, we will retain this data for ten years from the month in which your enquiry or complaint is closed. We will also request that data is deleted by SurveyMonkey during this retention period. After ten years, we will remove any links to identifying information from our systems. During this time, you are entitled to request access to your personal data under Data Protection legislation. You are also entitled to request the correction of inaccurate data. Any such requests or concerns should be sent to the Data Protection Officer at the below address.
