Hywel Dda University Health Board
Mr X complained that he had not received a response to a complaint raised in September 2021.
In considering this complaint the Ombudsman was concerned that the Health Board had failed to conclude its investigation in line with Putting Things Right guidelines.
It has agreed to conclude and issue its response no later than 9 July 2022. The Ombudsman was prepared to accept this resolution as an alternative to investigating the complaint.
